Generate FDR form

Why?

For the FDR outcomes 'FDR exempt' and 'FDR completed' you must print and sign an FDR form and give it to all the parties. This form is required as proof if the parties decide to go to Family Court.

How?

  1. Open the decision record from within the event.

  2. Select the Generate Template button.

    Key point!

    If your decision type doesn’t require an FDR form (for example, ‘Refer to Preparation for Mediation’), you won’t be able to generate one.

  3. The form will open as a PDF, so you can print it.

    Tip!

    • Depending on what browser you use, you may be asked whether you wish to open or save the PDF.
    • The form can be generated at any time, so there is no need to save it.

What next?

Once you have completed the decision and given the FDR form to the relevant parties, you should finalise the event.
